Telmisartan, Amlodipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the deal with Twynsta (which combines telmisartan and amlodipine). The amlodipine part is cleared by a liver enzyme called CYP3A4. In animal studies, high doses of atractylodes seem to slow down a similar enzyme, which could in theory let amlodipine build up a bit. If that happened, you might notice more of amlodipine's usual effects, like swollen ankles, dizziness, flushing, or blood pressure dropping lower than expected.

I want to be honest with you though: this is only a theoretical concern from lab and animal work, and it has not been shown in people. It's rated minor. Still, if you take Twynsta and want to try atractylodes, just give your pharmacist or doctor a heads-up first so they can keep an eye on your blood pressure.
