Lepirudin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the risk of bleeding when used concomitantly with anticoagulant and antiplatelet drugs.
Laboratory research suggests that atractylenolides II and III, constituents of atractylodes, reduce platelet activation. So far, this has not been shown in humans.

Lepirudin is a powerful blood thinner given by IV in the hospital, usually for people who can't take heparin. It works directly to block clotting, so anything that adds even a little extra anti-clotting action is worth watching. The concern here is simple: atractylodes might gently nudge bleeding risk higher if it's taken alongside Lepirudin.

That said, this is a theoretical worry. The only evidence comes from lab studies showing two atractylodes compounds calm platelets a bit. This hasn't been shown in real people. Still, with a strong thinner like Lepirudin, signs like easy bruising, nosebleeds, or unusual bleeding would matter. Since Lepirudin is hospital-managed, let your care team know about any supplements before they're given.
