Telmisartan, Amlodipine falls under Antihypertensive Drugs — the group Fenugreek interacts with.

Fenugreek may also have an additive effect on blood pressure-lowering medications. Studies on animals have shown that fenugreek seed can decrease both systolic and diastolic blood pressure by up to 22% when combined with metoprolol. Therefore, it is essential to monitor your blood pressure regularly if you are taking fenugreek and metoprolol together or any other antihypertensive drugs.

Twynsta combines two blood pressure medicines: telmisartan (which relaxes blood vessels by blocking angiotensin) and amlodipine (a calcium channel blocker that also widens vessels). Fenugreek, when taken in concentrated supplement doses, may gently nudge your blood pressure down on its own. Stacked on top of Twynsta, that could add up to a little more lowering than you expect.

I want to keep this in perspective. The evidence here is weak and mostly comes from animal studies, so it is a minor, possible interaction, not something to panic about. The spice amount you sprinkle in cooking is very unlikely to matter. With supplement doses, just watch for signs your pressure dipped too low: dizziness, lightheadedness when standing, or feeling unusually tired. Check your readings at home and chat with your pharmacist or doctor before starting fenugreek capsules.
