Benzthiazide falls under Antihypertensive Drugs — the group Fenugreek interacts with.

Fenugreek may also have an additive effect on blood pressure-lowering medications. Studies on animals have shown that fenugreek seed can decrease both systolic and diastolic blood pressure by up to 22% when combined with metoprolol. Therefore, it is essential to monitor your blood pressure regularly if you are taking fenugreek and metoprolol together or any other antihypertensive drugs.

Benzthiazide (Exna) is a thiazide-type water pill that lowers blood pressure and helps your body shed extra fluid. The thinking here is pretty simple: fenugreek may nudge your blood pressure down a little on its own, so stacking it on top of a blood pressure medicine could add up to a slightly bigger drop than you expect. In plain terms, you might feel more lightheaded, dizzy, or wobbly when you stand up quickly.

The good news is this is rated a minor interaction, and the evidence is weak, mostly animal studies, not strong human trials. A sprinkle of fenugreek in your cooking is very unlikely to matter. Concentrated supplement doses are where I'd be a bit more watchful. If you do take both, just check your blood pressure now and then, and chat with your pharmacist or doctor first.
