Why this applies to Naratriptan

Naratriptan falls under QT INTERVAL-PROLONGING DRUGS — the group Grapefruit interacts with.

Grapefruit or grapefruit juice, especially if consumed in large amounts, can cause additive QT interval prolongation when taken with QT interval-prolonging drugs, potentially increasing the risk of ventricular arrhythmias.
Clinical research in healthy volunteers shows that drinking 6 liters of grapefruit juice over 6 hours prolonged the QTc by a peak amount of 14 milliseconds (ms). This prolongation was similar to the QT prolongation caused by the drug moxifloxacin. In individuals with long QT syndrome, a smaller dose of grapefruit juice, 1.5 liters, resulted in a greater peak QTc prolongation of about 30 ms. The effect of smaller quantities of grapefruit juice on the QT interval is unclear.

Here's the thing to know about pairing grapefruit with naratriptan (Amerge): this isn't about grapefruit changing how your body breaks the drug down. It's about both of them nudging your heart's electrical rhythm in the same direction, what we call QT prolongation. Triptans like naratriptan can already affect the heart a bit, and very large amounts of grapefruit juice can add to that, in theory raising the chance of an irregular heartbeat.

But context matters. The study showing real QT changes used huge amounts (think liters of juice in a few hours), far more than a normal serving with breakfast. An occasional half grapefruit is very unlikely to cause trouble for most people. If you have a heart rhythm condition, long QT, or take other heart medicines, please run it by your pharmacist or doctor first.

Naratriptan tablets are used to treat migraine headaches — with or without aura — when a migraine is already happening. It is a rescue treatment, meaning you take it during an attack, not daily to prevent migraines.

It is not used to prevent migraines, and it has not been proven effective for cluster headaches. Naratriptan should only be used when a doctor has clearly confirmed a diagnosis of migraine.
