QT INTERVAL-PROLONGING DRUGS
1 of 3Why this applies to Sertraline
Sertraline falls under QT INTERVAL-PROLONGING DRUGS — the group Grapefruit interacts with.
Clinical research in healthy volunteers shows that drinking 6 liters of grapefruit juice over 6 hours prolonged the QTc by a peak amount of 14 milliseconds (ms). This prolongation was similar to the QT prolongation caused by the drug moxifloxacin. In individuals with long QT syndrome, a smaller dose of grapefruit juice, 1.5 liters, resulted in a greater peak QTc prolongation of about 30 ms. The effect of smaller quantities of grapefruit juice on the QT interval is unclear.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's something many people don't expect about grapefruit and sertraline (Zoloft). The concern isn't that grapefruit changes how your body breaks the drug down. Instead, both can nudge your heart's electrical timing in the same direction, slightly lengthening what's called the QT interval. When two things push that way together, the small risk of an irregular heartbeat can add up.
That said, the studies behind this used very large amounts of grapefruit juice (think liters, not a slice with breakfast). A normal serving of grapefruit is unlikely to cause trouble for most healthy people. But if you have a known heart rhythm problem, long QT, or take other heart-affecting medicines, this matters more. Please run it by your pharmacist or doctor before making grapefruit a daily habit.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 3Sertraline also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the gist for Zoloft (sertraline): grapefruit can block a liver enzyme called CYP3A4, which is one of the pathways your body uses to clear this medicine. When that pathway slows down, sertraline can build up a bit higher than expected. In practice that might mean more of the usual side effects, things like nausea, drowsiness, jitteriness, trouble sleeping, or upset stomach.
The good news is sertraline is not a prodrug, and it actually leans on several enzymes, not just CYP3A4, so for most people the bump is modest. Still, the interaction is rated as significant, and the effect can linger up to two days after drinking the juice. If you enjoy grapefruit regularly, just run it by your pharmacist or doctor first, especially if you're older or take other medicines.

SERTRALINE (Zoloft)
3 of 3Clinical research shows that grapefruit juice inhibits the cytochrome P450 3A4 (CYP3A4) metabolism of sertraline, increasing blood levels of sertraline.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: grapefruit can slow down one of the liver enzymes (called CYP3A4) that helps clear sertraline from your body. When that pathway is blocked, more sertraline can hang around in your bloodstream than expected. Sertraline is the active drug itself, not a prodrug, so higher levels usually mean a stronger effect.
In practice that could mean more of sertraline's usual side effects, like nausea, headache, trouble sleeping, jitteriness, or feeling more drowsy. The evidence here comes from a lower quality study, so the effect is possible but not guaranteed, and an occasional grapefruit is different from drinking the juice every day. Don't change anything on your own. Just check with your pharmacist or doctor about how much grapefruit, if any, is okay for you.

Sertraline treats a range of mental health conditions. All formulations are approved for major depressive disorder (MDD) in adults and obsessive-compulsive disorder (OCD). The oral solution (including Zoloft oral solution) and several tablet formulations are also approved for panic disorder, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD).
For OCD specifically, the capsule formulations and some tablet products are approved for use in adults and children aged 6 and older. Sertraline hydrochloride tablets (plain, not film-coated) are approved for OCD in pediatric patients as well. However, sertraline is not approved for major depressive disorder in children — only in adults.
