Diosmin + Trimipramine
Trimipramine brand names: Surmontil, Trimip, Rhotrimine, Tripramine, Trimp.
Diosmin has a moderate interaction with Trimipramine. Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
Why this applies to Trimipramine
Trimipramine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: trimipramine is partly broken down by a liver enzyme called CYP2C9, and diosmin may slow that enzyme down a bit. If trimipramine clears more slowly, more of it can hang around in your blood. With a tricyclic like this, that could mean stronger side effects such as drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat. Trimipramine is one of those drugs where the dose needs to sit in a fairly narrow window, so even modest changes are worth watching.
I want to be honest about the evidence, though. This is theoretical, based on how diosmin behaves in the lab, not proven with trimipramine itself. Don't change anything on your own. Just give your pharmacist or doctor a heads-up so they can keep an eye out.

Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Trimipramine page
“If you take trimipramine (Surmontil), a tricyclic antidepressant, read this list carefully. Nine of the 139 supplements and herbs on file are rated major, and most touch the heart: several, grapefruit among them, can stretch the QT interval, the heart's electrical reset time, and raise the risk of a dangerous rhythm.”

About Diosmin
What is Diosmin?
- Chronic venous insufficiency
- Hemorrhoids
- Leg swelling and heaviness
- Varicose veins
- Spider veins
Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. Talk to your pharmacist or doctor before starting it, especially if you are pregnant or breastfeeding.
About Trimipramine
What Is Trimipramine Used For?
Brand names: Surmontil, Trimip, Rhotrimine, Tripramine, Trimp
Trimipramine capsules are used to relieve the symptoms of depression. Both supported products (Trimipramine and Trimipramine Maleate oral capsules) carry this same use.
Depression with an “endogenous” pattern is more likely to improve than other depressive states. In the label’s studies, it worked about as well as amitriptyline in less-depressed outpatients, but somewhat less well in more severely depressed ones. In hospitalized patients, it worked about as well as imipramine.

Trimipramine Is Linked to Lower Levels of 3 Nutrients
Natural Medicines records Trimipramine as a medicine that can lower these. This is not an interaction with Diosmin — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
