Ibuprofen falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body relies on an enzyme called CYP2C9 to break down ibuprofen. Diosmin may slow that enzyme down a bit. If ibuprofen clears more slowly, levels could creep up and stay around longer, which can mean a stronger drug effect and a higher chance of the usual ibuprofen side effects, like stomach upset, heartburn, or in some people fluid retention or higher blood pressure.
I want to be honest about the strength of this one. It's theoretical, based on how the drugs behave and on a study using a related medicine (diclofenac), not on ibuprofen itself. For most folks taking occasional ibuprofen, this likely won't be a big deal. Still, if you use it regularly, it's worth a quick chat with your pharmacist or doctor.

A case of spontaneous intraventricular hemorrhage has been reported for a 77-year-old female after 6 weeks of warfarin therapy, despite an international normalized ratio (INR) of only 1.8. The patient had also been taking aspirin and diosmin for several years. Experts speculate that chronic intake of diosmin predisposed the patient to spontaneous intraventricular hemorrhage by inducing chronic microcirculatory hypertension and inhibiting platelet aggregation. The presence of aspirin was also thought to play a role in this event.

Here's the thing with ibuprofen and diosmin together: ibuprofen is an NSAID, and on its own it can make your blood a little less sticky and irritate the stomach lining, which raises bleeding risk. Diosmin may have a mild effect on platelets (the cells that help blood clot) too. Put them together and, in theory, you could nudge your bleeding risk up a bit.
I want to be honest about how thin the evidence is here. This concern comes mostly from a single case report involving a much stronger combination (warfarin plus aspirin plus diosmin), not ibuprofen alone. For most people the risk is low. Still, if you notice easy bruising, nosebleeds, black or bloody stools, or stomach pain, check in with your pharmacist or doctor before continuing.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. What Is Ibuprofen Used For?

Over-the-counter ibuprofen products (tablets, capsules and others) temporarily relieve minor aches and pains from headache, toothache, backache, muscular aches, the common cold, menstrual cramps and minor arthritis pain. They also temporarily reduce fever.
Prescription ibuprofen tablets are used for signs and symptoms of rheumatoid arthritis and osteoarthritis, mild to moderate pain, and primary dysmenorrhea (painful periods). Caldolor (intravenous) is used in adults and children 3 months and older for mild to moderate pain, moderate to severe pain along with opioid medicines, and fever. Ibuprofen Lysine (intravenous) is used only to close a patent ductus arteriosus (PDA, a heart vessel that stays open) in certain premature infants.
