Why this applies to Nifedipine
Nifedipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Diosmin interacts with.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Your body clears nifedipine using a liver enzyme called CYP3A4. Diosmin may gently block that same enzyme. If it does, nifedipine could hang around in your bloodstream a little longer than usual, which means a bit more of its effect. With nifedipine, that could show up as lower blood pressure, dizziness or lightheadedness (especially when standing), flushing, headache, swollen ankles, or a faster heartbeat.
I want to be honest about the evidence though. This is mostly theoretical, and lab studies actually disagree on whether diosmin affects CYP3A4 at all. The clearest signal comes from a different drug, carbamazepine, not nifedipine. So this isn't a reason to panic. Just give your pharmacist or doctor a heads-up before adding diosmin, and watch for extra dizziness.

Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. Nifedipine treats vasospastic angina (chest pain caused by a spasm in a heart artery) and chronic stable angina (chest pain that comes on with effort). It can be used for chronic stable angina when other treatments, such as beta blockers or nitrates, haven't worked well enough or can't be tolerated.
Nifedipine extended-release tablets, including Procardia XL, are also used to treat high blood pressure. The nifedipine capsule label covers the angina uses only.
