Why this applies to Phenytoin
Phenytoin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body clears phenytoin (Dilantin) partly through a liver enzyme called CYP2C9. Diosmin may slow that enzyme down a bit, so in theory phenytoin could clear more slowly and creep up in your blood. Since phenytoin is a narrow therapeutic index drug (the safe window between too little and too much is small), even modest changes can matter.
If levels rose, you might notice things like unsteady walking, dizziness, slurred speech, double vision, or drowsiness. To be clear, this concern is theoretical and based on how the enzyme works, not on real-world phenytoin studies, so don't panic. Just loop in your pharmacist or doctor before adding diosmin. They can decide whether a phenytoin blood level check makes sense.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. What Is Phenytoin Used For?

Phenytoin treats seizures. Oral products are used for generalized tonic-clonic (grand mal) seizures and for psychomotor (temporal lobe) or complex partial seizures. Most oral products are also used to prevent and treat seizures during or after neurosurgery.
Phenytoin Sodium Injection is used for generalized tonic-clonic status epilepticus (a prolonged, dangerous seizure emergency). It is also used for seizures during neurosurgery. The injection should be used only when taking phenytoin by mouth is not possible. Dilantin-125 and Phenytoin oral suspension are listed for tonic-clonic and psychomotor seizures.
