Why this applies to Prednisolone

Prednisolone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help clear prednisolone out of your system. Diosmin may slow that enzyme down a bit. If it does, prednisolone could linger longer and build up, which can nudge its steroid effects and side effects upward. Think things like fluid retention and puffiness, higher blood sugar, mood or sleep changes, stomach upset, or easier bruising.

That said, I want to be honest about the evidence. This is mostly theoretical and based on lab and animal work, with the human data coming from a different drug (carbamazepine), not prednisolone itself. So a big problem is unlikely for most folks. Still, it's worth a quick word with your pharmacist or doctor before combining them, especially on long-term steroid use.

Prednisolone also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, diosmin might increase levels of drugs that are substrates of P-glycoprotein (P-gp).
Preliminary laboratory research suggests that diosmin inhibits P-gp. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of fexofenadine 120 mg increases blood levels of fexofenadine, a P-gp substrate, by approximately 49% and decreases the apparent oral clearance of fexofenadine by 41%. The time taken to reach maximum plasma concentration, the half-life, and the apparent renal clearance of fexofenadine are not affected. For this reason, it is speculated that diosmin inhibits P-gp in the intestine, but not in the kidney or liver.

Here's the gist: prednisolone is one of many medicines your body can move around using a transport protein called P-glycoprotein (P-gp). Diosmin appears to slow that pump down, at least in the gut. In theory, that could let a bit more prednisolone get absorbed and hang around in your system, which might mean slightly stronger steroid effects, things like fluid retention, a bump in blood sugar, stomach upset, mood changes, or trouble sleeping.

I want to be honest about the strength of this, though. It's based on lab and animal work, not on real-world studies pairing diosmin with prednisolone, so any effect is likely small and may not show up at all. Still, it's worth a quick mention to your pharmacist or doctor before you combine them.

Prednisolone is prescribed for many different conditions where inflammation or an overactive immune system is causing harm. These range from severe allergic reactions and asthma to certain skin diseases, blood disorders, eye problems, and some cancers.

It is also used to support the body when the adrenal glands aren't making enough of their own steroids, and to manage flares of conditions like ulcerative colitis, multiple sclerosis, and nephrotic syndrome (a kidney condition causing protein loss in the urine).
