Tapentadol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

Here's the honest picture: this concern is mostly theoretical. Diosmin may slow down a liver enzyme called CYP2C9, and the worry is that drugs cleared by that enzyme could build up. The thing is, tapentadol leans only a little on CYP2C9. It's handled mainly by a different process (glucuronidation), so this particular pathway plays a small role for it.

Tapentadol is an active opioid, not a prodrug, so if levels did edge up, you might notice more of its usual effects: drowsiness, dizziness, nausea, constipation, or slowed breathing. I wouldn't lose sleep over this combo, but since tapentadol is a strong pain medicine, it's smart to mention the diosmin to your pharmacist or doctor and watch for extra sleepiness when starting out.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. About Tapentadol

What Is Tapentadol Used For?

Brand names: Nucynta, Nucynta ER

Tapentadol tablets (including Nucynta and Tapentadol Hydrochloride tablets) are used for severe acute pain in adults when other treatments are not adequate. Nucynta tablets are also approved for acute pain in children aged 6 and older who weigh at least 40 kg.

Nucynta ER and tapentadol extended-release tablets are used for severe, ongoing pain that requires around-the-clock opioid treatment and cannot be managed with other options, including severe neuropathic pain (nerve pain) caused by diabetic peripheral neuropathy in adults. Extended-release formulations are not intended for as-needed use.
