Sildenafil Citrate falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

Here's the gist with Viagra and diosmin. Sildenafil is partly broken down by a liver enzyme called CYP2C9, and diosmin may slow that enzyme down a bit. If that happens, sildenafil could hang around longer in your body and feel a little stronger than usual. In real terms, that might mean more of sildenafil's typical side effects: headache, facial flushing, a stuffy nose, indigestion, or feeling lightheaded, especially when you stand up.

Now, the honest part: this is a theoretical concern based on how these compounds behave, not on studies done with sildenafil itself. For most people it likely won't cause real trouble. Still, if you take both and notice stronger side effects, or if you also take blood pressure or nitrate medicines, check in with your pharmacist or doctor first.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the gist: your body clears sildenafil (Viagra) mainly through a liver enzyme called CYP3A4. Diosmin may gently slow that same enzyme down. If it does, sildenafil could hang around a bit longer and feel stronger than usual. With a PDE inhibitor like this, that can mean more of the typical side effects, things like headache, facial flushing, a stuffy nose, dizziness, or a bigger drop in blood pressure.

The good news is that this is mostly theoretical. The clearest evidence comes from a different drug (carbamazepine), and lab studies on diosmin and CYP3A4 actually disagree with each other. So this is a possible, not a proven, concern for sildenafil. To be safe, let your pharmacist or doctor know you take both, and don't change anything on your own.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. What Is Sildenafil Used For?

Brand name: Viagra

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
