Why this applies to Diazepam

Diazepam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

Here's the gist: diosmin may gently slow down one of the liver enzymes (called CYP2C9) that helps clear diazepam from your body. If that happens, a little more diazepam could linger in your blood than usual. Diazepam isn't a prodrug, so more of it in your system means its calming effects could be stronger, things like extra drowsiness, dizziness, feeling foggy, or being unsteady on your feet.

I want to keep this in perspective, though. This concern is theoretical, based mostly on how diosmin behaved with a different drug, not from studies on diazepam itself. So it's a maybe, not a sure thing. Still, if you take diazepam regularly and want to add diosmin, it's worth a quick chat with your pharmacist or doctor first, especially if you already feel sleepy on it.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the gist: your body uses an enzyme called CYP3A4 to help break down diazepam (Valium). Diosmin may slow that enzyme down a bit. If it does, diazepam could clear more slowly and build up in your system. Since diazepam is an active calming drug (not a prodrug that needs switching on), more of it usually means stronger effects, things like extra drowsiness, dizziness, feeling foggy or unsteady, or sleeping more than usual.

I want to keep this in perspective, though. The evidence here is fairly thin and mixed, with the clearest signal coming from a different drug (carbamazepine), not diazepam. For many people this won't cause a noticeable problem. Still, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you're older or already feel sleepy on diazepam. Don't change anything on your own.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. What Is Diazepam Used For?

  • Diazepam is used to relieve anxiety and to control agitation caused by alcohol withdrawal. It is also used along with other medications to control muscle spasms and spasticity caused by certain neurological disorders such as cerebral palsy (condition that causes difficulty with movement and balance), paraplegia (inability to move parts of the body), athetosis (abnormal muscle contractions), and stiff-man syndrome (a rare disorder with muscle rigidity and stiffness). Diazepam is also used along with other medications to control seizures. Diazepam is in a class of medications called benzodiazepines. It works by calming abnormal overactivity in the brain.
