Why this applies to Morphine Sulfate

Morphine Sulfate falls under P-GLYCOPROTEIN SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might increase levels of drugs that are substrates of P-glycoprotein (P-gp).
Preliminary laboratory research suggests that diosmin inhibits P-gp. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of fexofenadine 120 mg increases blood levels of fexofenadine, a P-gp substrate, by approximately 49% and decreases the apparent oral clearance of fexofenadine by 41%. The time taken to reach maximum plasma concentration, the half-life, and the apparent renal clearance of fexofenadine are not affected. For this reason, it is speculated that diosmin inhibits P-gp in the intestine, but not in the kidney or liver.

Here's the gist: morphine is moved around your body partly by a transporter called P-glycoprotein (P-gp). Diosmin appears to block that transporter in the gut, at least in lab and small human studies. In theory, that could let a bit more morphine get absorbed and build up in your bloodstream. If that happens, you might notice stronger opioid effects, things like extra drowsiness, dizziness, constipation, nausea, or in a worst case slowed breathing.

I want to be honest about the strength of this, though. The evidence here is mostly theoretical and based on test-tube and animal work, plus one study using a different drug. It may not amount to much in real life. Still, since morphine is a powerful medicine, please check with your pharmacist or doctor before adding diosmin so they can keep an eye on how you feel. Don't change your morphine on your own.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. About Morphine

What Is Morphine Used For?

Brand names: MS Contin, Kadian, Avinza, Morphabond, Arymo ER

Morphine is used for pain that is severe enough to need an opioid and that other treatments cannot control well. It is meant for people who have not done well on, or cannot tolerate, other options.

Some uses depend on the product. Morphine sulfate tablets treat acute pain in adults and in children weighing at least 50 kg, and chronic pain in adults. Morphine oral solution (2 mg/mL and 4 mg/mL) treats acute and chronic pain in adults and acute pain in children 2 years and older. The 20 mg/mL oral solution is only for opioid-tolerant adults. Extended-release capsules and tablets, including MS Contin, are for severe, persistent pain that needs daily, long-term opioid treatment. Duramorph and preservative-free morphine injection can be given by vein, or into the epidural or intrathecal space (around the spinal cord). Infumorph and Mitigo are only for continuous epidural or intrathecal infusion for intractable chronic pain.
