Mometasone, formoterol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

Here's the honest take on pairing diosmin with your Zenhale inhaler (mometasone and formoterol). The concern on file is that diosmin may block a liver enzyme called CYP2C9, which could let certain medicines build up. But this worry is really theoretical, and it was studied with an oral drug (diclofenac), not an inhaled one.

With Zenhale you breathe the medicine right into your lungs, so very little reaches your liver to begin with. That makes a meaningful interaction unlikely. If anything ever did shift, formoterol effects to watch for would be a fast or pounding heartbeat, shakiness, or feeling jittery. I wouldn't lose sleep over this combo, but it never hurts to mention your supplements to your pharmacist or doctor.

Mometasone, formoterol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Zenhale combines mometasone (an inhaled steroid) with formoterol (a long-acting airway opener), and the mometasone part is broken down by the CYP3A4 enzyme. The idea here is that diosmin might slow that enzyme down a bit, which could let a little more steroid linger in your system than usual. In theory, that could nudge up steroid-related effects over time, things like mouth thrush, hoarseness, or with heavy long-term exposure, the usual steroid concerns.

Honestly, I wouldn't lose sleep over this one. The evidence for diosmin blocking CYP3A4 is mixed and mostly from lab and animal work, and an inhaled medication delivers only small amounts into the body. Still, since everyone's situation differs, run it past your pharmacist or doctor before adding diosmin.
