Why this applies to Acetaminophen

Acetaminophen falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2E1 substrates.
Chlorzoxazone is metabolized by CYP2E1 enzymes. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of chlorzoxazone (Paraflex 250) 250 mg increases blood levels of chlorzoxazone by 34% and decreases chlorzoxazone clearance by 40%. It is speculated that diosmin reduces the metabolism of chlorzoxazone by inhibiting CYP2E1.

Here is the gist. A small part of how your body handles acetaminophen (Tylenol) runs through an enzyme called CYP2E1. Diosmin may slow that enzyme down a little. In theory, slowing it could change how a tiny fraction of acetaminophen gets processed. The good news is that this is based on pharmacology reasoning and one study using a different muscle relaxant, not on real-world reports with Tylenol itself.

For everyday use, this is unlikely to cause problems, and acetaminophen at normal doses is generally well tolerated. The bigger thing to remember with acetaminophen is simply not exceeding the daily limit, since that is what strains the liver. If you take diosmin regularly, just mention it to your pharmacist so they can double-check with you.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. Acetaminophen temporarily relieves minor aches and pains caused by headaches, the common cold, backaches, muscle aches, toothaches, minor arthritis pain, and premenstrual or menstrual cramps. It also temporarily reduces fever.

These uses apply across oral tablet, suspension, and oral liquid products. The intravenous (IV) injection form is used in clinical settings to treat acute pain and fever, including in pediatric patients 2 years and older.
