Why this applies to Triamcinolone

Triamcinolone falls under P-GLYCOPROTEIN SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might increase levels of drugs that are substrates of P-glycoprotein (P-gp).
Preliminary laboratory research suggests that diosmin inhibits P-gp. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of fexofenadine 120 mg increases blood levels of fexofenadine, a P-gp substrate, by approximately 49% and decreases the apparent oral clearance of fexofenadine by 41%. The time taken to reach maximum plasma concentration, the half-life, and the apparent renal clearance of fexofenadine are not affected. For this reason, it is speculated that diosmin inhibits P-gp in the intestine, but not in the kidney or liver.

Here's the gist: diosmin may gently block a gut transporter called P-glycoprotein, which is one of the pumps that helps push certain drugs back out of your body. Triamcinolone is considered a substrate of that pump, so in theory, taking diosmin could let a bit more triamcinolone get absorbed and raise its levels slightly. If that happened, you might notice more of the usual steroid effects, like fluid retention, higher blood sugar, mood changes, or trouble sleeping.

I'll be honest with you though, this is mostly lab-based speculation, not something proven in real patients on triamcinolone. The signal comes from a study with a different drug (fexofenadine). For most people this likely won't be a big deal, but it's worth a quick chat with your pharmacist or doctor before adding diosmin, especially with longer steroid courses.

What Is Triamcinolone Used For?

Brand names: Aristocort, Kenacort, Adcortyl Intra-articular / Intradermal, Kenalog Intra-articular / Intramuscular, Adcortyl in Orabase, Triesence

Triamcinolone is available in many forms — injections, creams, ointments, nasal sprays, dental pastes, and eye injections — each approved for specific uses. Injectable forms given into muscle are used when oral steroids aren't an option, covering severe allergies (like asthma flares, severe skin reactions, drug reactions, and serum sickness), serious skin diseases, certain endocrine (hormonal) disorders, and more.

Injections given directly into a joint (intra-articular) or soft tissue help with conditions like rheumatoid arthritis, gout flares, bursitis, and tendon inflammation. Injections into skin lesions (intralesional) treat conditions such as keloids, alopecia areata, and psoriatic plaques. Triesence (an ophthalmic injection) is specifically indicated for eye conditions like uveitis and temporal arteritis, and for visualization during eye surgery. Topical creams, ointments, lotions, and sprays relieve itching and inflammation from corticosteroid-responsive skin conditions. Dental paste forms (Oralone, Kourzeq) treat painful mouth sores from injury. Nasal sprays temporarily relieve hay fever and upper respiratory allergy symptoms such as runny nose, sneezing, and congestion.
