Why this applies to dabigatran

dabigatran falls under P-GLYCOPROTEIN SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might increase levels of drugs that are substrates of P-glycoprotein (P-gp).
Preliminary laboratory research suggests that diosmin inhibits P-gp. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of fexofenadine 120 mg increases blood levels of fexofenadine, a P-gp substrate, by approximately 49% and decreases the apparent oral clearance of fexofenadine by 41%. The time taken to reach maximum plasma concentration, the half-life, and the apparent renal clearance of fexofenadine are not affected. For this reason, it is speculated that diosmin inhibits P-gp in the intestine, but not in the kidney or liver.

Here's the thing to know about dabigatran (Pradaxa): your gut relies on a little pump called P-glycoprotein to limit how much of it gets absorbed into your bloodstream. Diosmin appears to slow that pump down, at least in early lab and small human studies. If less of the drug gets pumped back out, more can end up in your blood, which could nudge dabigatran's blood-thinning effect a bit higher.

For a blood thinner, that mainly means a possibly higher chance of bruising, nosebleeds, or bleeding that's slow to stop. To be clear, this is mostly theoretical and based on a related drug, not on dabigatran directly, so I don't want you alarmed. Still, since bleeding risk is serious, please run this combo by your pharmacist or doctor before starting diosmin.

Theoretically, diosmin may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
A case of spontaneous intraventricular hemorrhage has been reported for a 77-year-old female after 6 weeks of warfarin therapy, despite an international normalized ratio (INR) of only 1.8. The patient had also been taking aspirin and diosmin for several years. Experts speculate that chronic intake of diosmin predisposed the patient to spontaneous intraventricular hemorrhage by inducing chronic microcirculatory hypertension and inhibiting platelet aggregation. The presence of aspirin was also thought to play a role in this event.

Here's the simple version: dabigatran (Pradaxa) is already a blood thinner, and diosmin may have a mild blood-thinning effect of its own. Put them together and, in theory, you could tip slightly toward easier bleeding. With dabigatran that might show up as bruising more than usual, nosebleeds, bleeding gums, or blood in your urine or stool.

I want to be honest about how solid this is. The concern comes mostly from a single case report involving warfarin and aspirin, not dabigatran, so the evidence here is weak and largely theoretical. Still, since dabigatran is a serious medication, don't add diosmin on your own. Run it by your pharmacist or doctor first, and let them know right away if you notice any unusual bleeding.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. About Dabigatran

What Is Dabigatran Used For?

Brand names: Pradaxa, Pradax

Dabigatran etexilate capsules are used in adults to reduce the risk of stroke and dangerous blood clots in people with non-valvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). They are also used to treat deep venous thrombosis (DVT — a blood clot in a deep vein) and pulmonary embolism (PE — a blood clot in the lungs), to lower the risk of those clots coming back, and to prevent DVT and PE in adults who have had hip replacement surgery.

In children ages 8 to under 18, dabigatran etexilate capsules are used to treat venous thromboembolic events (VTE — blood clots in the veins) and to reduce the risk of those clots recurring. Pradaxa Oral Pellets — a separate formulation — are indicated for the treatment and prevention of recurrent VTE in younger children ages 3 months to under 12 years.
