Why this applies to dabigatran

dabigatran falls under P-GLYCOPROTEIN SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of P-glycoprotein substrates.

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the thing to know about Pradaxa (dabigatran): your gut uses a little pump called P-glycoprotein to push some of the drug back out, which limits how much gets into your blood. Quercetin, in concentrated supplement doses, may block that pump. If it does, more dabigatran could be absorbed, and since this is a blood thinner, that could mean a higher chance of bruising, nosebleeds, or more serious bleeding.

I'll be honest, this is mostly theoretical and based on weak studies of other drugs, not dabigatran itself. The quercetin in foods like onions, apples, and berries is very unlikely to matter. But because dabigatran leaves little room for error, please check with your pharmacist or doctor before adding a high-dose quercetin supplement.

Quercetin is a plant flavonoid with antioxidant and anti-inflammatory properties found in many common foods and sold as a supplement. While early research is interesting for allergies, blood pressure, and inflammation, strong human evidence is limited for most uses. It is generally well tolerated in food amounts, but talk to your pharmacist or doctor before taking high-dose supplements.

About Dabigatran

What Is Dabigatran Used For?

Brand names: Pradaxa, Pradax

Dabigatran etexilate capsules are approved for adults to reduce the risk of stroke and systemic embolism (blood clots traveling through the body) in non-valvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). Capsules are also used to treat deep vein thrombosis (DVT — a blood clot in a deep vein, usually the leg) and pulmonary embolism (PE — a clot in the lungs), to prevent those clots from coming back, and to prevent DVT and PE in adults who have had hip replacement surgery.

For children, the capsule form is approved for treating VTE (venous thromboembolism — blood clots in veins) and reducing recurrence in patients aged 8 to under 18. Pradaxa Oral Pellets are approved specifically for children aged 3 months to under 12 years for the same treatment and recurrence-prevention purposes.
