Why this applies to Levonorgestrel

Levonorgestrel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the gist if you're thinking about taking diosmin around the same time as levonorgestrel (Plan B and similar). Levonorgestrel is cleared from your body partly by a liver enzyme called CYP3A4. Diosmin may mildly slow that enzyme down, which could in theory let levonorgestrel hang around a little longer. With this drug, that would mostly mean a touch more of its usual effects, like nausea, breast tenderness, headache, or some spotting and changes to your next period.

Honestly, the evidence here is thin and mixed. Most of it comes from lab studies and one study using a different drug (carbamazepine), not levonorgestrel. For a one-time emergency pill, this is unlikely to be a real problem. Still, if you use a levonorgestrel IUD or take diosmin regularly, it's worth a quick chat with your pharmacist or doctor.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. Talk to your pharmacist or doctor before starting it, especially if you are pregnant or breastfeeding.

About Levonorgestrel

What Is Levonorgestrel Used For?

Brand names: Plan B, Mirena, Levonelle One Step, Levonelle 1500, Norgeston, Skyla, Liletta, Kyleena

Levonorgestrel is used in two very different ways depending on the product. As an intrauterine system (IUS), Mirena and Liletta prevent pregnancy for up to 8 years and also treat heavy menstrual bleeding for up to 5 years in people who choose intrauterine contraception. Kyleena prevents pregnancy for up to 5 years, and Skyla prevents pregnancy for up to 3 years — neither Kyleena nor Skyla is indicated for heavy menstrual bleeding.

As an oral tablet, products such as AfterPill, AfterPlan, Amazon Basic Care Levonorgestrel, Econ Morning After, Emergency Contraceptive, and Empowered Choice are used as emergency (backup) contraception to reduce the chance of pregnancy after unprotected sex or contraceptive failure. The oral tablet is not intended for routine birth control.
