CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Rifampin
Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Diosmin interacts with.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this one. Diosmin may gently block a liver enzyme called CYP3A4, which helps clear many medicines. Rifampin happens to be handled by that enzyme too, so in theory diosmin could slow how fast rifampin leaves your body and nudge its blood levels up. That might mean a bit more chance of stomach upset, or changes in liver enzyme tests your doctor watches during TB treatment.
I'll be honest with you though, this is mostly theoretical. The lab data is mixed, and the clearest signal involves a different drug (carbamazepine), not rifampin. Rifampin is also famous for the opposite effect, it speeds up the breakdown of many other drugs. Before combining these, just run it by your pharmacist or doctor so they can keep an eye on things.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. What Is Rifampin Used For?
Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor
Rifampin treats all forms of tuberculosis. It is always used as part of a combination of TB medicines, not on its own.
It also treats asymptomatic meningococcal carriers. These are people who carry the bacteria Neisseria meningitidis in the nose and throat without being sick. Rifampin is not used to treat an actual meningococcal infection.
Rifampin capsules are taken by mouth. Rifadin IV and Rifampin for Injection are given through a vein for the initial treatment and retreatment of tuberculosis when the medicine cannot be taken by mouth.
