Tamoxifen Citrate falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing worth knowing about tamoxifen: it's actually a prodrug. Your body has to convert it into its active form (called endoxifen), and the enzymes that do that work include CYP2C9 and, more importantly, CYP2D6. Diosmin may gently slow down CYP2C9, which in theory could shift how tamoxifen is processed. Because tamoxifen depends on those enzymes to become active, anything that interferes with them could in principle affect how well it works.
I want to be straight with you though: this is a theoretical concern based on lab and pharmacology data, mostly studied with a different drug (diclofenac), not tamoxifen itself. We don't have real-world evidence that diosmin weakens tamoxifen. Still, since tamoxifen is an important cancer medicine, please run it by your oncologist or pharmacist before adding diosmin.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the wrinkle with tamoxifen: it's actually a prodrug. Your body has to convert it into its active forms (mainly through enzymes like CYP3A4 and CYP2D6) before it does its job of fighting breast cancer. So this isn't a simple "levels go up" situation.
Diosmin may mildly slow CYP3A4, the enzyme that helps turn tamoxifen into its active metabolites. In theory, that could mean a bit less active drug forming, which is the part you actually want working. I want to be honest though: the evidence here is weak and conflicting (mostly lab and animal studies with a different drug, carbamazepine), so this is more theoretical than proven. Since tamoxifen is too important to gamble with, please run diosmin past your oncologist or pharmacist before starting it.

P-GLYCOPROTEIN SUBSTRATES

Preliminary laboratory research suggests that diosmin inhibits P-gp. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of fexofenadine 120 mg increases blood levels of fexofenadine, a P-gp substrate, by approximately 49% and decreases the apparent oral clearance of fexofenadine by 41%. The time taken to reach maximum plasma concentration, the half-life, and the apparent renal clearance of fexofenadine are not affected. For this reason, it is speculated that diosmin inhibits P-gp in the intestine, but not in the kidney or liver.

Here's the gist for tamoxifen and diosmin. Tamoxifen is moved around your body partly by a tiny pump in your gut called P-glycoprotein (P-gp). Lab work and one small study in healthy men hint that diosmin can slow that pump down. In theory, that could let a little more tamoxifen get absorbed, which might nudge its levels up.
Honestly, this is on the soft side of the evidence scale. It comes from test-tube research and a study using a different drug (fexofenadine), not tamoxifen itself, so we're really just speculating. If your levels did rise, you might notice more of tamoxifen's usual effects like hot flashes, leg cramps, or fluid retention. It's worth a quick chat with your pharmacist or oncologist before adding diosmin, but no need to panic.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. Talk to your pharmacist or doctor before starting it, especially if you are pregnant or breastfeeding.

What Is Tamoxifen Used For?
Brand names: Nolvadex, Soltamox
Tamoxifen is used to treat estrogen receptor-positive metastatic breast cancer in both women and men, and as follow-up (adjuvant) therapy after surgery for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.
In women with ductal carcinoma in situ (DCIS — an early, non-invasive breast change) following surgery and radiation, tamoxifen lowers the risk of developing invasive breast cancer. It is also approved to reduce the risk of breast cancer in women who are at high risk but have not yet been diagnosed. Both Tamoxifen Citrate tablets and Soltamox oral solution share these indications.
