Why this applies to Naproxen, Sumatriptan

Naproxen, Sumatriptan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

Treximet is a combo of naproxen (an anti-inflammatory pain reliever) and sumatriptan (a migraine medicine). The naproxen part is partly cleared by a liver enzyme called CYP2C9. On paper, diosmin may slow that enzyme down, which could let naproxen build up a little more than usual in your body. This idea comes mostly from a study with a similar drug (diclofenac), so it's theoretical for naproxen, not proven.

If naproxen levels did rise, you might notice more of its typical side effects, like stomach upset, heartburn, or, with longer use, irritation or bleeding in the gut. This is usually a mild concern, especially with occasional use, but it's worth a quick chat with your pharmacist or doctor before combining them, particularly if you take Treximet often.

Theoretically, diosmin may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
A case of spontaneous intraventricular hemorrhage has been reported for a 77-year-old female after 6 weeks of warfarin therapy, despite an international normalized ratio (INR) of only 1.8. The patient had also been taking aspirin and diosmin for several years. Experts speculate that chronic intake of diosmin predisposed the patient to spontaneous intraventricular hemorrhage by inducing chronic microcirculatory hypertension and inhibiting platelet aggregation. The presence of aspirin was also thought to play a role in this event.

Treximet combines naproxen (an NSAID) with sumatriptan (a migraine medicine). For this interaction, the part that matters is the naproxen, because NSAIDs can thin the blood a little and irritate the stomach lining, which already nudges up bleeding risk. Diosmin may gently affect platelets too, so stacking the two could, in theory, make bleeding or bruising slightly more likely.

I want to be honest about how shaky this evidence is. It comes mostly from a single case report involving warfarin and aspirin, not Treximet, so this is largely theoretical. Still, if you notice unusual bruising, nosebleeds, black or bloody stools, or stomach pain, check in with your pharmacist or doctor before changing anything.

What is Diosmin?

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies.
