Losartan Potassium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

Here's a wrinkle worth knowing about losartan. Your body actually has to convert losartan into its more powerful active form, and the enzyme CYP2C9 does a lot of that conversion work. Diosmin is thought to slow down that same enzyme. So in theory, if diosmin blocks CYP2C9, you could end up with a bit less of the active form of losartan, meaning slightly weaker blood pressure control rather than too much.

I'll be straight with you: this is a theoretical concern based on how the drug is processed, not something proven with losartan in real patients. The studies showing diosmin blocking this enzyme were done with a different drug. Most people won't notice anything, but if you take both, it's smart to keep an eye on your blood pressure readings and check in with your pharmacist or doctor.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's something interesting about losartan: it actually works a bit backwards from most drugs. Losartan is a prodrug, meaning your body has to convert it (largely through the CYP3A4 and CYP2C9 enzymes) into its stronger active form, sometimes called E-3174, which does most of the blood-pressure-lowering work.

Diosmin may mildly block CYP3A4. In theory, that could slightly reduce how much active form you make, which might make losartan a touch less effective at controlling your blood pressure. I want to be honest though: this is mostly theoretical and based on lab and animal studies (much of it with a different drug), and losartan has a backup enzyme pathway. The everyday risk looks low. Still, if you're starting diosmin, just give your pharmacist or doctor a heads-up and keep an eye on your blood pressure readings.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. Talk to your pharmacist or doctor before starting it, especially if you are pregnant or breastfeeding.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
