Of the 160 supplement and herbal ingredients on file, five carry a major rating, and those deserve understanding first. Grapefruit can raise sildenafil levels by blocking the liver enzyme that clears it, St. John's wort does the reverse and can leave the drug too weak to work, and hawthorn may deepen the drop in blood pressure.
The 114 moderate and 41 minor entries mostly change how the liver processes sildenafil through the CYP3A4 and CYP2C9 enzymes, nudging its level up or down. A few bulky or absorbent products can keep an oral dose from absorbing, so space those well apart.
