Why this applies to Sildenafil

Sildenafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the short version: this one barely registers. The thinking is that red clover's plant estrogens might slow down a liver enzyme called CYP2C9, and sildenafil is partly handled by that enzyme. If that slowdown actually happened, a little more sildenafil could hang around, which in theory might nudge up its usual effects like headache, flushing, a stuffy nose, or lightheadedness from lower blood pressure.

But the real-world evidence points the other way. The signal only shows up in lab dish studies, and when researchers actually tested red clover in people, it didn't change how the body handled a CYP2C9 drug. So I wouldn't lose sleep over this pairing. Still, since sildenafil already affects blood pressure, it's worth a quick word with your pharmacist or doctor before adding red clover.

Here's the gist for sildenafil (Viagra or Revatio): your liver uses an enzyme called CYP3A4 to clear it out of your system. Red clover contains plant compounds (isoflavones) that, in a test tube, looked like they might slow that enzyme down a little. In theory, slowing it could let sildenafil build up, which might mean more of its usual effects, like headache, facial flushing, a stuffy nose, or lightheadedness from blood pressure dipping.

The reassuring part: when researchers actually tested red clover in people, it did not meaningfully change how the body handled a similar CYP3A4 medication. So this is mostly a theoretical concern, rated minor and unlikely. Still, if you take sildenafil for heart or lung pressure (PAH), or you're on other blood pressure meds, it's worth a quick chat with your pharmacist before adding red clover.

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
