Wild Cherry and Sildenafil

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Sildenafil

Sildenafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Wild Cherry interacts with.

In vitro research suggests that wild cherry can inhibit cytochrome P450 3A4 (CYP3A4) enzymes. Theoretically, wild cherry might increase levels of drugs metabolized by CYP3A4. However, so far, this interaction has not been reported in humans.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and others.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your body relies on an enzyme called CYP3A4 to break down sildenafil. Lab studies (in test tubes, not people) hint that wild cherry might slow that enzyme down a bit. If it did, more sildenafil could hang around in your system, which in theory could nudge up its usual effects like headache, facial flushing, a stuffy nose, dizziness, or a bigger drop in blood pressure.

The important thing is that this is only a theoretical concern based on early lab work, and it has never actually been reported in real people. Normal cough-syrup or lozenge amounts of wild cherry are unlikely to cause trouble. Still, since sildenafil already affects blood pressure, it is worth a quick chat with your pharmacist or doctor before regularly combining the two.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

About Wild Cherry

What is Wild Cherry?

Wild Cherry
  • Cough
  • Bronchitis and respiratory irritation
  • Sore throat
  • Digestive upset and diarrhea
  • Flavoring agent

Wild cherry bark is a traditional cough remedy found in many old-fashioned cough syrups and lozenges, but there is little modern scientific evidence proving it works. The bark, leaves, and seeds naturally contain cyanide-producing compounds, so it should be used only in properly prepared products and avoided in large amounts. Talk with a pharmacist or doctor before using it, especially during pregnancy or for children.

About Sildenafil

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
