Cyclosporine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Wild Cherry interacts with.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with wild cherry and your cyclosporine. Cyclosporine is cleared from your body largely by a liver enzyme called CYP3A4. Lab studies (done in test tubes, not in people) suggest wild cherry may slow that enzyme down. If that happened in real life, your cyclosporine could build up to higher levels than expected.
Why does that matter? Cyclosporine has a narrow safety window, meaning small changes in its level can tip you into trouble. Higher levels could mean more kidney strain, high blood pressure, tremors, or headaches. The flip side is also true if levels swing the other way and your transplant protection weakens. The good news: this is only a theoretical concern so far, never actually reported in patients. Still, since cyclosporine is so dose-sensitive, please run any wild cherry product past your transplant team or pharmacist first.

Wild cherry bark is a traditional cough remedy found in many old-fashioned cough syrups and lozenges, but there is little modern scientific evidence proving it works. The bark, leaves, and seeds naturally contain cyanide-producing compounds, so it should be used only in properly prepared products and avoided in large amounts. What Is Cyclosporine Used For?
Brand names: Neoral, Gengraf, Sandimmune, Ciclosporine, Cequa, Verkazia, Vevye
Oral cyclosporine (such as Gengraf, Neoral and the modified generic capsules and solution) is used to prevent organ rejection in kidney, liver and heart transplants. It is also used for severe active rheumatoid arthritis that has not responded well enough to methotrexate. It is also used for severe, hard-to-treat plaque psoriasis in adults with normal immune systems.
Sandimmune capsules and injection, used with corticosteroids, prevent organ rejection and treat chronic rejection in people previously treated with other immune-suppressing medicines. Eye products have different uses. Restasis, Restasis MultiDose and generic cyclosporine eye emulsion increase tear production in dry eye linked to eye inflammation. Cequa increases tear production in keratoconjunctivitis sicca (dry eye). Vevye treats signs and symptoms of dry eye disease. Verkazia treats vernal keratoconjunctivitis in children and adults.
