Sildenafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Sage interacts with.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2C9. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for sage and sildenafil. Sildenafil is cleared from your body partly by a liver enzyme called CYP2C9. In lab dish studies, sage extracts seemed to slow that enzyme down a bit. If that happened in a real person, sildenafil could hang around longer and act a little stronger, which might mean more of its usual side effects like headache, facial flushing, a stuffy nose, dizziness, or a bigger drop in blood pressure.
The important part: this is only test-tube evidence, and it has never actually been reported in people. Normal cooking amounts of sage are very unlikely to matter. Concentrated sage capsules or the essential oil are the bigger question mark. To be safe, just mention any sage supplements to your pharmacist or doctor.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down and clear sildenafil. Lab studies suggest sage may slow that enzyme down a bit. If that happened in a person, sildenafil could hang around longer and build up, which can mean stronger effects and more of its usual side effects, things like headache, facial flushing, a stuffy nose, dizziness, or a bigger drop in blood pressure.
The honest part: this is only a theoretical concern based on test-tube work, and it hasn't actually been seen in real people. A pinch of sage in your cooking is very unlikely to matter. Concentrated sage extracts or capsules are where I'd be more cautious. If you take sildenafil regularly, just run your sage supplement by your pharmacist or doctor first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

What Is Sildenafil Used For?
Brand name: Revatio
Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.
Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
