Why this applies to Sildenafil

Sildenafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Black Seed interacts with.

Theoretically, black seed might increase levels of drugs metabolized by CYP2C9.
In vitro research suggests that thymoquinone, a constituent of black seed, can decrease the metabolism of phenytoin by a mechanism possibly related to the inhibition of CYP2C9. The effect of black seed on CYP2C9 is unclear. This has not been shown in humans.

Here's the gist with black seed and sildenafil. Your liver uses an enzyme called CYP2C9 to help clear sildenafil from your body. In lab dishes, a compound in black seed (thymoquinone) seemed to slow that enzyme down a bit. If that held true in people, sildenafil could linger longer and act a little stronger than expected, meaning more chance of its usual side effects like headache, flushing, a stuffy nose, dizziness, or low blood pressure.

I want to be honest, though: this is only a theoretical concern based on test-tube work, and it has never been shown in actual humans. Food-amount black seed in cooking is very unlikely to matter. Before adding concentrated black seed supplements, just run it by your pharmacist or doctor, especially if you take sildenafil for lung blood pressure.

Animal research shows that black seed reduces the total systemic exposure to sildenafil by 43%. So far, this interaction has not been reported in humans.

Here's the gist: in animal studies, black seed seemed to lower the amount of sildenafil that reached the bloodstream by about 43%. Sildenafil works as-is (it's not a prodrug that needs activating), so having less of it floating around could mean a weaker effect. For erectile dysfunction that might look like a less reliable response, and for pulmonary hypertension it could theoretically mean the medicine doesn't control symptoms as well.

I'd keep this in perspective, though. This is theoretical and based only on animal data, with nothing reported in people, so it's far from certain. A pinch of black seed in cooking is unlikely to matter; concentrated supplement doses are the bigger question. If you're considering a black seed supplement, just check with your pharmacist or doctor first.

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
