Sildenafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Coleus interacts with.

Theoretically, taking coleus may affect drugs metabolized by CYP2C9 and increase the risk of adverse effects or reduce the effectiveness.
Research on the effect of coleus on CYP2C9 is conflicting. Some animal research shows that coleus extract can induce CYP2C9, while in vitro research shows that coleus can inhibit CYP2C9. Until more is known, advise patients that taking coleus might increase or decrease levels of drugs metabolized by CYP2C9.

Here's the gist with Coleus and sildenafil: your liver uses an enzyme called CYP2C9 to help clear sildenafil from your body. Coleus might tweak that enzyme, but honestly the science is messy. Some lab work suggests it speeds the enzyme up (which could make sildenafil clear faster and work less well), while other lab work suggests it slows the enzyme down (which could let sildenafil build up and cause more side effects like headache, flushing, a stuffy nose, dizziness, or a bigger drop in blood pressure).

This is theoretical and based on test-tube and animal data, so I wouldn't lose sleep over it. Still, since Coleus itself can lower blood pressure, the combo could stack that effect. Run it by your pharmacist or doctor before pairing them, especially if you take sildenafil for pulmonary hypertension.

Sildenafil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, taking coleus might decrease serum levels of drugs metabolized by CYP3A4.
In vitro research shows that coleus can activate the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of coleus and other drugs affected by these enzymes.

Here's the gist for sildenafil (Viagra or Revatio). Your liver clears this medication mainly through an enzyme called CYP3A4. Lab studies suggest Coleus may nudge that enzyme to work a bit faster. If that happens in real life, your body could break down sildenafil more quickly, leaving less of it around. The practical worry would be a weaker effect, so an erection might be harder to achieve, or for pulmonary hypertension, the medication might not control your symptoms as well.

I want to be honest though: this is based only on test-tube research, not on people, so it may not matter at all. Sildenafil is not a prodrug, so the direction holds. Just mention the Coleus to your pharmacist or doctor before combining them.

Coleus is a plant from the mint family whose root contains a compound called forskolin, often marketed for weight loss, asthma, and heart health. While early lab and small human studies are interesting, solid proof that Coleus supplements help any of these conditions is limited. What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
