Indomethacin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Coleus interacts with.

Theoretically, taking coleus may affect drugs metabolized by CYP2C9 and increase the risk of adverse effects or reduce the effectiveness.
Research on the effect of coleus on CYP2C9 is conflicting. Some animal research shows that coleus extract can induce CYP2C9, while in vitro research shows that coleus can inhibit CYP2C9. Until more is known, advise patients that taking coleus might increase or decrease levels of drugs metabolized by CYP2C9.

Here's the deal with combining Coleus and indomethacin. Your liver uses an enzyme called CYP2C9 to help clear indomethacin out of your body. Coleus (and its active compound forskolin) might bump into that same enzyme, but honestly, the science here is murky. Some animal studies suggest Coleus speeds the enzyme up, while lab-dish studies suggest it slows it down. So we genuinely can't say which direction this goes.

If it slowed clearance, indomethacin could build up, raising the chance of stomach upset, ulcers, bleeding, or kidney strain. If it sped clearance up, your pain relief might fade. Since indomethacin is a regular drug (not a prodrug), and this is only theoretical lab data, I wouldn't panic, but please run it past your pharmacist or doctor first.

Coleus is a plant from the mint family whose root contains a compound called forskolin, often marketed for weight loss, asthma, and heart health. While early lab and small human studies are interesting, solid proof that Coleus supplements help any of these conditions is limited. About Indomethacin

What Is Indomethacin Used For?

Brand names: Indocin, Indocin SR, Indo-Lemmon, Indomethagan, Tivorbex

Indomethacin oral suspension and rectal suppository forms (including Indocin oral suspension and Indocin suppositories) are used to treat moderate to severe rheumatoid arthritis (including acute flares), moderate to severe ankylosing spondylitis (spine inflammation), moderate to severe osteoarthritis, acute painful shoulder (bursitis and/or tendinitis), and acute gouty arthritis. Oral capsules — both immediate-release and extended-release — share these same approved uses, except that extended-release capsules are not approved for acute gouty arthritis.

Indomethacin for Injection (intravenous) is a completely separate use: it is given to premature infants to close a hemodynamically significant patent ductus arteriosus (a blood vessel near the heart that should close after birth but hasn't) when standard medical management has not worked.
