Why this applies to Sildenafil
Sildenafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your body clears sildenafil mainly through a liver enzyme called CYP3A4. The thinking here is that ashwagandha may gently speed up that enzyme, which could clear sildenafil a little faster and make it slightly weaker. In real terms, that might mean a softer or shorter response if you take it for erectile dysfunction, or less benefit if you use it (as Revatio) for pulmonary hypertension.
Here's the important part: this concern comes only from lab and test-tube studies, not from people, so it's theoretical and rated minor. For most folks it likely won't be noticeable. Still, if you take sildenafil for a lung condition, or you start ashwagandha and feel it's not working as well, check in with your pharmacist or doctor first.

Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. What Is Sildenafil Used For?
Brand name: Revatio
Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.
Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
