Why this applies to Sildenafil

Sildenafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the gist for sildenafil (Viagra or Revatio). Goldenseal contains alkaloids like berberine and hydrastine that, in test tube studies, can mildly slow down a liver enzyme called CYP2C9, which helps clear sildenafil from your body. If that enzyme works slower, sildenafil could linger a bit longer and feel stronger. That might mean more of its usual side effects, like headache, facial flushing, a stuffy nose, dizziness, or a drop in blood pressure.

The important catch: this is only theoretical, based on lab data, and has not actually been shown to happen in people. So I wouldn't lose sleep over it. Still, since lightheadedness and low blood pressure can matter, it's worth a quick chat with your pharmacist or doctor before pairing the two.

Sildenafil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist: your body relies on an enzyme called CYP3A4 to break down sildenafil. Goldenseal appears to slow that enzyme down. When the cleanup crew works slower, more sildenafil can linger in your bloodstream than usual. Sildenafil isn't a prodrug, so this means a stronger, more drawn-out effect, along with a higher chance of its typical side effects: headache, flushing, a stuffy nose, dizziness, and most importantly a bigger drop in blood pressure.

This is rated a moderate, possible interaction backed by some human research, so it's worth taking seriously but not panicking over. Casual amounts likely matter less than concentrated goldenseal capsules. Before combining them, please check with your pharmacist or doctor first.

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
