Why this applies to Sildenafil

Sildenafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rabdosia Rubescens interacts with.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the gist for sildenafil (Viagra or Revatio). Your liver clears this drug mainly through an enzyme called CYP3A4. Lab and animal studies suggest that oridonin, a compound in Rabdosia rubescens, may speed that enzyme up. If that happens in people, your body could break down sildenafil faster, leaving less in your bloodstream.

In real terms, that might mean a weaker response, a softer or shorter erection if you take it for ED, or less benefit for breathing and exercise if you take Revatio for pulmonary hypertension. I want to be honest though: this is only test-tube and animal evidence, and it has never actually been reported in humans, so the risk is uncertain. Before adding this herb, just check with your pharmacist or doctor.

Sildenafil also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 2C9 (CYP2C9) enzyme. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the gist for sildenafil (Viagra or Revatio). Your body partly clears this drug using a liver enzyme called CYP2C9. Lab studies suggest a compound in Rabdosia rubescens, called oridonin, may speed up that enzyme. If that happened in real life, your body could clear sildenafil a bit faster, which might make it work less well. For erectile dysfunction that could mean a weaker response, and for pulmonary hypertension it could mean less symptom control.

The important part: this is only test-tube research, and it has never actually been reported in people. So I wouldn't lose sleep over it. Still, since safety of this herb isn't well studied, it's worth a quick chat with your pharmacist or doctor before combining them.

About Sildenafil

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
