L-citrulline and Sildenafil

Here is the interaction we found between these two, with the evidence behind it.

Moderate

PHOSPHODIESTERASE-5 INHIBITORS

Why this applies to Sildenafil

Sildenafil falls under PHOSPHODIESTERASE-5 INHIBITORS — the group L-citrulline interacts with.

Theoretically, concurrent use of phosphodiesterase-5 (PDE-5) inhibitors and L-citrulline might result in additive vasodilation.
L-citrulline is converted to L-arginine, which can increase nitric oxide and cause vasodilation. Theoretically, taking L-arginine with PDE-5 inhibitors might have additive vasodilatory and hypotensive effects. However, in studies evaluating the combined use of L-arginine and sildenafil for erectile dysfunction, hypotension was not reported.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind with this pairing. Sildenafil already relaxes and widens your blood vessels, which is how it helps with erections or eases pressure in the lungs. L-citrulline goes down a similar road: your body turns it into L-arginine, which boosts nitric oxide and also relaxes blood vessels. So in theory, stacking the two could add up and nudge your blood pressure down a little more than you'd expect.

That said, this concern is theoretical, based on how the two work rather than strong proof. In actual studies combining L-arginine with sildenafil, low blood pressure wasn't reported. Still, if you notice dizziness, lightheadedness, headache, or feeling faint, that's worth a heads-up. Please check with your pharmacist or doctor before combining them, especially if you take other blood pressure or nitrate medicines.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About L-citrulline

What is L-citrulline?

L-citrulline
  • Exercise performance
  • Muscle soreness and fatigue
  • Blood flow and circulation
  • Erectile dysfunction
  • Blood pressure support

L-citrulline is an amino acid that the body turns into L-arginine to help make nitric oxide, which relaxes blood vessels and may improve blood flow. It is popular for exercise performance and circulation, but the evidence is mixed and most benefits are modest. It appears generally well tolerated, but talk to your pharmacist or doctor before starting, especially if you have a health condition or are pregnant.

About Sildenafil

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
