Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group L-citrulline interacts with.

Theoretically, concomitant use of L-citrulline with antihypertensive drugs might have additive effects and increase the chance of hypotension.
L-citrulline is converted to L-arginine, which can increase nitric oxide and cause vasodilation. However, a meta-analysis of 5 small clinical studies suggests that taking L-citrulline 3-6 grams daily for 1-8 weeks does not lower blood pressure when compared with control.

Here's the thing about pairing L-citrulline with iloprost (Ventavis): both can relax and widen your blood vessels. Iloprost is already a vasodilator that you inhale to ease the high pressure in your lung arteries, and citrulline turns into L-arginine, which nudges your body to make more nitric oxide and open vessels a bit more. Stacked together, the worry is that your blood pressure could dip too low.

If that happened, you might feel dizzy, lightheaded, flushed, headachy, or faint, which are effects iloprost can cause on its own. Honestly, the evidence here is pretty soft. Studies suggest citrulline supplements don't reliably lower blood pressure much, so this is more theoretical than proven. Still, please run it by your pharmacist or doctor before adding it.

Aurlumyn (iloprost injection, given intravenously) is approved to treat severe frostbite in adults, specifically to reduce the risk of losing fingers or toes (digit amputations). Its effectiveness was established in young, healthy adults who experienced frostbite at high altitudes.
