Captopril falls under ANTIHYPERTENSIVE DRUGS — the group L-citrulline interacts with.

Theoretically, concomitant use of L-citrulline with antihypertensive drugs might have additive effects and increase the chance of hypotension.
L-citrulline is converted to L-arginine, which can increase nitric oxide and cause vasodilation. However, a meta-analysis of 5 small clinical studies suggests that taking L-citrulline 3-6 grams daily for 1-8 weeks does not lower blood pressure when compared with control.

Here's the gist with captopril: it lowers your blood pressure by relaxing blood vessels. L-citrulline works on the same idea, since your body turns it into L-arginine, which makes nitric oxide and can also relax those vessels. Stack the two together and, in theory, your pressure could dip a little lower than expected. If that happens, you might notice dizziness, lightheadedness, fatigue, or feeling faint when you stand up quickly.

The reassuring part: this is mostly a theoretical concern. Several small studies found L-citrulline didn't actually lower blood pressure much on its own, so for most people the effect is minor. Still, it's worth a quick chat with your pharmacist or doctor before starting it, especially if your pressure already runs on the low side.

What Is Captopril Used For?

Brand names: Capoten, Acepril

Captopril tablets are used to treat high blood pressure (hypertension), heart failure, and left ventricular dysfunction (weakened pumping function of the heart) in stable patients after a heart attack. For the heart attack indication, it helps improve survival and reduces the chance of developing full heart failure.

Captopril tablets are also used to slow the progression of diabetic nephropathy (kidney damage caused by diabetes) in people with type 1 (insulin-dependent) diabetes who have retinopathy and significant protein in their urine. It reduces the risk of the kidneys failing to the point of needing dialysis or a transplant.
