L-citrulline and Olmesartan Medoxomil

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIHYPERTENSIVE DRUGS

Why this applies to Olmesartan Medoxomil

Olmesartan Medoxomil falls under ANTIHYPERTENSIVE DRUGS — the group L-citrulline interacts with.

Theoretically, concomitant use of L-citrulline with antihypertensive drugs might have additive effects and increase the chance of hypotension.
L-citrulline is converted to L-arginine, which can increase nitric oxide and cause vasodilation. However, a meta-analysis of 5 small clinical studies suggests that taking L-citrulline 3-6 grams daily for 1-8 weeks does not lower blood pressure when compared with control.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version. Olmesartan (Benicar) already lowers your blood pressure by relaxing your blood vessels. L-citrulline turns into L-arginine in your body, which can make more nitric oxide and relax those vessels a little too. So in theory, stacking the two could nudge your pressure down further than expected.

The good news is the evidence is weak. A review of several small studies found L-citrulline (around 3 to 6 grams a day) didn't actually lower blood pressure much on its own, so a big drop is unlikely for most folks. Still, if you notice dizziness, lightheadedness when standing, or feeling unusually tired, those can be signs your pressure dipped too low. Don't start or stop anything on your own. Check with your pharmacist or doctor first, especially if your blood pressure already runs low.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

“Olmesartan medoxomil (Benicar) lowers blood pressure by relaxing blood vessels, and nearly everything on this page, 119 of the 136 ingredients, pulls on that same lever, deepening the drop or working against it. Four entries rate major, which is why this list deserves genuine care rather than a quick skim.”

About L-citrulline

What is L-citrulline?

L-citrulline
  • Exercise performance
  • Muscle soreness and fatigue
  • Blood flow and circulation
  • Erectile dysfunction
  • Blood pressure support

L-citrulline is an amino acid that the body turns into L-arginine to help make nitric oxide, which relaxes blood vessels and may improve blood flow. It is popular for exercise performance and circulation, but the evidence is mixed and most benefits are modest. It appears generally well tolerated, but talk to your pharmacist or doctor before starting, especially if you have a health condition or are pregnant.

About Olmesartan

What Is Olmesartan Used For?

Brand names: Olmesartan Medoxomil, Benicar

Olmesartan — sold as Benicar and under the generic names olmesartan medoxomil and olmesartan medoxomil-hydrochlorothiazide — is used to treat high blood pressure (hypertension) in adults and in children six years of age and older. It can be used alone or combined with other blood pressure medicines.

Lowering high blood pressure with olmesartan reduces the risk of serious cardiovascular events, primarily stroke and heart attack. Managing blood pressure is most effective as part of a broader plan that may include diet, exercise, and other therapies your doctor recommends.
