Sildenafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist: your body clears sildenafil mainly through a liver enzyme called CYP3A4. In test-tube studies, horny goat weed seems to slow that same enzyme down. If that happened in real life, sildenafil could hang around longer and hit a bit harder, meaning more of its usual effects like headache, flushing, a stuffy nose, dizziness, or a bigger drop in blood pressure.

One thing worth knowing: horny goat weed is also marketed as a mild PDE5-type herb, the same general action as sildenafil, so stacking them could add up. That said, this enzyme effect has only been seen in the lab, never confirmed in people, so I wouldn't panic. Just run it by your pharmacist or doctor before combining them, especially if you have heart or blood pressure concerns.

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
