Why this applies to Sildenafil

Sildenafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist: your body clears sildenafil partly using a liver enzyme called CYP2C9, and milk thistle might slow that enzyme down a bit. If it did, sildenafil could hang around longer than usual, which in theory could bump up its typical effects like headache, facial flushing, a stuffy nose, indigestion, or a dip in blood pressure that leaves you lightheaded.

The honest part: the evidence here is shaky and mixed. Some lab work hints at this, but better-quality human studies suggest milk thistle doesn't really affect CYP2C9 much. So for most people this is unlikely to cause a real problem. Still, if you take sildenafil for ED or for pulmonary hypertension, it's worth a quick chat with your pharmacist or doctor before adding milk thistle.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Your body relies on a liver enzyme called CYP3A4 to clear sildenafil out of your system. The thinking here is that milk thistle might slow that enzyme down a little, which could let sildenafil linger longer and feel stronger. With sildenafil, that would most likely mean more of its usual effects, like headache, facial flushing, a stuffy nose, indigestion, or lightheadedness from a bigger drop in blood pressure.

Honestly, though, this is a very weak, mostly theoretical concern. Good human studies found milk thistle did not meaningfully change how the body handles similar CYP3A4 drugs, and the evidence here comes from lab and animal work. A real-world problem with sildenafil is unlikely. Still, it never hurts to mention both to your pharmacist or doctor, especially if you notice stronger side effects.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
