Why this applies to Remdesivir

Remdesivir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the situation with these two. Diosmin may slow down an enzyme in your body called CYP3A4, and remdesivir is one of the medicines that enzyme helps clear. In theory, if that enzyme works slower, remdesivir levels could edge up a bit, which might mean a little more chance of side effects like nausea or changes in liver-related blood tests.

But honestly, I wouldn't lose sleep over this one. The evidence is mostly theoretical and even conflicting (some research shows diosmin doesn't touch CYP3A4 at all). On top of that, remdesivir is given as a short IV course in a hospital or clinic setting where you're monitored. Still, it's always smart to tell your care team about any supplement, including diosmin, so they have the full picture.

P-GLYCOPROTEIN SUBSTRATES

Remdesivir also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, diosmin might increase levels of drugs that are substrates of P-glycoprotein (P-gp).
Preliminary laboratory research suggests that diosmin inhibits P-gp. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of fexofenadine 120 mg increases blood levels of fexofenadine, a P-gp substrate, by approximately 49% and decreases the apparent oral clearance of fexofenadine by 41%. The time taken to reach maximum plasma concentration, the half-life, and the apparent renal clearance of fexofenadine are not affected. For this reason, it is speculated that diosmin inhibits P-gp in the intestine, but not in the kidney or liver.

Here's the gist: remdesivir is moved around the body in part by a transporter called P-glycoprotein (P-gp), and diosmin may block that transporter a little, at least in the gut. In theory, slowing P-gp could let more of a drug stay in the body. So the concern is that diosmin might nudge remdesivir levels up somewhat.

In real life, I wouldn't lose sleep over this one. Remdesivir is given as an IV in a hospital or clinic setting, so the gut effect diosmin showed in studies likely matters very little here. The evidence is also weak, coming mostly from lab work and one study on a different drug. Still, it's worth mentioning diosmin to your care team so they have the full picture.

What Is Remdesivir Injection Used For?

Brand name: Veklury

Veklury (remdesivir) is given by IV infusion to treat COVID-19 caused by the SARS-CoV-2 virus. It is approved for adults and for children from birth through age 17 who weigh at least 1.5 kg (about 3.3 lbs).

Veklury can be used for patients who are hospitalized with COVID-19, or for patients who are not hospitalized but have mild-to-moderate COVID-19 and are at high risk of getting worse — including being hospitalized or dying from the disease.
