Why this applies to Aspirin, Omeprazole

Aspirin, Omeprazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

Here's the gist with Yosprala (the aspirin plus omeprazole combo) and diosmin. Diosmin may gently block a liver enzyme called CYP2C9. Omeprazole is partly handled by that enzyme, so in theory diosmin could slow how fast your body clears it, letting omeprazole levels drift a little higher. The aspirin portion isn't really cleared this way, so it's mostly the omeprazole side we'd watch.

Honestly, this is a theoretical concern drawn from lab work and a study using a different drug, not from real-world reports with this product. Any extra omeprazole effect would likely be mild (think more acid suppression, occasional headache or stomach upset). I wouldn't lose sleep over it, but do mention the diosmin to your pharmacist or doctor so they can keep an eye on things.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the gist for Yosprala, which packs aspirin together with omeprazole. Diosmin might slow down a liver enzyme called CYP3A4. In theory, if a drug leans on that enzyme to be cleared, diosmin could let it build up a bit. The catch is that the evidence here is pretty thin and conflicting. It's mostly based on lab work plus one small study using carbamazepine, not your specific medication.

Honestly, neither aspirin nor omeprazole depends heavily on CYP3A4, so I wouldn't expect a meaningful problem from this combo. Still, if you ever notice extra stomach upset, easy bruising, or unusual bleeding, give your pharmacist or doctor a heads-up before changing anything. A quick conversation is the safest move if you're starting diosmin.

Theoretically, diosmin may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
A case of spontaneous intraventricular hemorrhage has been reported for a 77-year-old female after 6 weeks of warfarin therapy, despite an international normalized ratio (INR) of only 1.8. The patient had also been taking aspirin and diosmin for several years. Experts speculate that chronic intake of diosmin predisposed the patient to spontaneous intraventricular hemorrhage by inducing chronic microcirculatory hypertension and inhibiting platelet aggregation. The presence of aspirin was also thought to play a role in this event.

Here's the gist with Yosprala (the aspirin plus omeprazole combo): the aspirin half is what we care about for this interaction. Aspirin already thins your blood a little by keeping platelets from clumping together. Diosmin, the citrus flavonoid in your vein supplement, may gently nudge in the same direction, so stacking them could theoretically raise your bleeding risk a bit.

Honestly, the evidence here is thin, just a single older case report involving a woman who was also on warfarin. So I wouldn't panic. But do keep an eye out for things like easy bruising, nosebleeds, bleeding gums, or dark stools, and loop in your pharmacist or doctor before starting diosmin so they can weigh it for you.
