Why this applies to Avatrombopag Maleate

Avatrombopag Maleate falls under P-GLYCOPROTEIN SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might increase levels of drugs that are substrates of P-glycoprotein (P-gp).
Preliminary laboratory research suggests that diosmin inhibits P-gp. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of fexofenadine 120 mg increases blood levels of fexofenadine, a P-gp substrate, by approximately 49% and decreases the apparent oral clearance of fexofenadine by 41%. The time taken to reach maximum plasma concentration, the half-life, and the apparent renal clearance of fexofenadine are not affected. For this reason, it is speculated that diosmin inhibits P-gp in the intestine, but not in the kidney or liver.

Here's the short version: avatrombopag (Doptelet) is carried by a transporter called P-glycoprotein, and diosmin may gently block that transporter in your gut. If it does, a bit more of the drug could get absorbed, which in theory could nudge your platelet-boosting effect higher. Avatrombopag isn't a prodrug, so more drug means more effect, not less.

Now, I want to keep this in perspective. This concern comes only from lab and early human studies (with a different drug), so it's mostly theoretical. Still, since avatrombopag works on platelets and clotting, it's worth a quick chat with your doctor or pharmacist before adding diosmin, especially around the time of a procedure.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. What Is Avatrombopag Used For?

Brand name: Doptelet

Avatrombopag treats thrombocytopenia (a low platelet count that raises bleeding risk) in three groups of patients. Doptelet tablets are used in adults with chronic liver disease who need their platelet count raised before a medical procedure, and in adults with chronic immune thrombocytopenia (ITP) whose platelet count has not responded well enough to earlier treatments. Doptelet tablets are also used in children aged 6 years and older with persistent or chronic ITP who have not responded to prior treatment.

Doptelet Sprinkle (oral granules) is specifically indicated for children aged 1 year to less than 6 years with persistent or chronic ITP who have not responded adequately to a previous treatment.
