Why this applies to Famotidine, Ibuprofen

Famotidine, Ibuprofen falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

The ibuprofen part of Duexis is cleared by a liver enzyme called CYP2C9. Diosmin may slow that enzyme down a bit, which could let ibuprofen build up slightly and linger longer in your body. The famotidine half is not really affected here, so it's the ibuprofen we're thinking about.

If ibuprofen levels run higher than usual, you might notice more of its typical downsides, like stomach upset, heartburn, fluid retention, higher blood pressure, or in rarer cases kidney strain. I'll be honest though: this is a theoretical concern based mostly on a study with a similar drug (diclofenac), not direct ibuprofen data. Casual diosmin use likely matters little, but if you take it daily, just run it by your pharmacist or doctor first.

Theoretically, diosmin may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
A case of spontaneous intraventricular hemorrhage has been reported for a 77-year-old female after 6 weeks of warfarin therapy, despite an international normalized ratio (INR) of only 1.8. The patient had also been taking aspirin and diosmin for several years. Experts speculate that chronic intake of diosmin predisposed the patient to spontaneous intraventricular hemorrhage by inducing chronic microcirculatory hypertension and inhibiting platelet aggregation. The presence of aspirin was also thought to play a role in this event.

Here's the thing to keep in mind with Duexis: the ibuprofen part already nudges up your bleeding risk on its own. Ibuprofen is an NSAID, so it makes platelets a bit less sticky and can irritate the stomach lining, which is exactly why famotidine is bundled in to help protect your gut. Adding diosmin to the mix is thought to theoretically push bleeding risk a little higher, since some lab work suggests it may slow platelet clumping too.

I want to be honest, though: this concern rests on very weak, anecdotal evidence (mostly one case involving warfarin and aspirin), not solid studies. Still, it's worth a quick mention to your pharmacist or doctor, especially if you notice easy bruising, dark stools, or stomach upset. Please don't stop either one on your own.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. Talk to your pharmacist or doctor before starting it, especially if you are pregnant or breastfeeding.
