Formoterol Fumarate, Glycopyrrolate falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture: the interaction on file is about diosmin slowing down a liver enzyme called CYP2C9. That matters for drugs cleared by CYP2C9, like diclofenac. But Bevespi (formoterol and glycopyrrolate) is an inhaler, and neither of these medicines relies heavily on CYP2C9 to be removed from your body. So this particular concern doesn't line up well with how these inhaled drugs actually work.
On top of that, this warning is theoretical, based on pharmacology rather than real-world reports with this inhaler. I wouldn't expect diosmin to noticeably change your breathing treatment or cause extra side effects like a fast heartbeat, jitteriness, or dry mouth. Still, it's always smart to run your full list, including supplements, past your pharmacist or doctor.
