Here's the simple version: your sinus product already contains pseudoephedrine, which is a stimulant that speeds up the heart and tightens blood vessels. Ephedra (ma huang) contains very similar stimulants, ephedrine and pseudoephedrine. So pairing them is like doubling up on the same kind of pressor effect, and that's why this combination is flagged as major.

Stacked together, these can push your blood pressure up and cause a racing or pounding heartbeat, jitteriness, headache, trouble sleeping, and in serious cases dangerous heart rhythm or stroke. The evidence here is mostly anecdotal, but the concern is very real because both work the same way. Keep in mind ephedra is actually banned in U.S. supplements. Please don't combine these, and check with your pharmacist or doctor about safer sinus options.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the thing I'd actually worry about with this combo, and it isn't really the enzyme part. The lab-based concern on file is that ephedra might speed up a liver enzyme called CYP1A2, which could clear certain drugs a little faster. But that's based on animal and test-tube data only, and acetaminophen and pseudoephedrine aren't mainly handled by that pathway. So that piece is mostly theoretical and minor here.

The bigger issue is overlap. Your product already contains pseudoephedrine, a stimulant, and ephedra packs ephedrine, a very similar one. Stacking them can push blood pressure and heart rate up and cause jitteriness, palpitations, headache, or trouble sleeping. Ephedra is banned in U.S. supplements for good reason. Please run this by your pharmacist before combining anything.

Here's the concern with combining ephedra and a product like Tylenol Sinus or Sudafed Sinus. This medicine pairs acetaminophen with pseudoephedrine. The worry is the liver. Acetaminophen, even at normal doses, puts some stress on your liver, and ephedra supplements have been linked to a number of liver injury cases. Put together, they could theoretically add up and be harder on your liver.

I'll be honest, this is based on how these substances behave rather than solid studies pairing them, so it's a theoretical caution. There's also a second issue: pseudoephedrine and ephedra are both stimulants, so taking them together could raise your heart rate and blood pressure more than expected. Ephedra is actually banned in U.S. supplements. Please talk with your pharmacist before using it with anything.
