STIMULANT DRUGS

Acetaminophen, Diphenhydramine, Pseudoephedrine falls under STIMULANT DRUGS — the group Ephedra interacts with.
Drugs with CNS stimulant properties might increase the risk of hypertension and the adverse cardiovascular effects of ephedra.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the big concern: this combo product already contains pseudoephedrine, which is itself a close cousin of the stimulants in ephedra. Adding ephedra on top is like doubling down on the gas pedal. Both push your 'fight or flight' system, so stacking them can stack the effects too.
Together they may drive your blood pressure up, make your heart race or pound, and leave you feeling jittery, anxious, or unable to sleep. In serious cases that strain on the heart and blood vessels has been linked to dangerous events. This warning is based mostly on case reports rather than hard studies, but the direction is clear and the risk is real. Ephedra is also banned in U.S. supplements. Please talk with your pharmacist or doctor before combining them.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
2 of 3Acetaminophen, Diphenhydramine, Pseudoephedrine also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture: there's a lab and animal hint that ephedra might speed up a liver enzyme called CYP1A2. In theory, that could clear out certain medicines a little faster. But this combo product (acetaminophen, diphenhydramine, and pseudoephedrine) isn't mainly handled by that enzyme, so the CYP1A2 effect probably won't change how it works much. The evidence here is weak and theoretical, so I wouldn't lose sleep over the enzyme part.
The bigger concern is overlap, not enzymes. Ephedra and the pseudoephedrine in this product are both stimulants. Together they can stack up and push your heart rate and blood pressure higher, leaving you jittery, anxious, or unable to sleep. Ephedra is also banned in U.S. supplements for safety reasons. HEPATOTOXIC DRUGS

There are numerous cases of liver toxicity from ephedra-containing supplements.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to watch with this combo. Your cold medicine already contains pseudoephedrine, which is a stimulant much like the ephedrine in ephedra. Stacking ephedra on top would be doubling down on those stimulant effects, so you could see a racing heart, jitteriness, a spike in blood pressure, or trouble sleeping. That's a real concern, separate from the liver issue.
On the liver side, both ephedra and the acetaminophen (Tylenol) in this product have been tied to liver stress. This particular warning is theoretical, based on how each behaves rather than proven cases of them together, but ephedra alone has caused liver injury. Since ephedra is banned in the U.S. anyway, please talk with your pharmacist about safer options.
