QT INTERVAL-PROLONGING DRUGS

Amitriptyline, Perphenazine falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the concern with this combination. Triavil pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic), and both of these can stretch out the heart's electrical recovery time, what doctors call the QT interval. Ephedra, with its stimulant ephedrine, has been shown to nudge the QT interval in the same direction even at usual doses in healthy people.
When you stack two QT-stretching effects together, the worry is a dangerous, irregular heartbeat. You might also notice a pounding or racing heart, dizziness, or feeling faint. Honestly, ephedra is banned in U.S. supplements for good reason, so I wouldn't use it at all. Please talk with your pharmacist or doctor about safer choices before combining anything with Triavil.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Triavil combines amitriptyline (an antidepressant) with perphenazine (an antipsychotic). The concern on file is that ephedra may speed up a liver enzyme called CYP1A2, which helps clear these medicines. In theory, that faster clearance could lower the levels in your blood, making the combo work less well. So you might notice your mood, sleep, or symptom control slipping. Keep in mind this idea comes only from lab and animal studies, so it's far from proven in people.
Honestly, the bigger worry here is ephedra itself. It's a banned, risky stimulant, and pairing a stimulant with these medicines can stress the heart and raise blood pressure. Please don't use ephedra, and talk with your pharmacist or doctor about safer options.
